Simplício started his playing career with his hometown team São Paulo, where he played from 2000 to 2004.
He then left São Paulo to join Parma of the Italian Serie A on a free transfer, after having been discovered by Arrigo Sacchi, Parma's football director at the time. He played two seasons Parma and became one of the team's most important players.
In June 2006, he was signed by U.S. Città di Palermo, also of Serie A.[1][2] His teammate Mark Bresciano also followed him to move to the Sicilian side. Simplício was priced €7.1 million but only €4.6 million to Parma.[3]
In January 2010, his agent Gilmar Rinaldi announced Simplício would not extend his contract with Palermo, set to expire in June 2010. He would instead look to join another team in the 2010–11 season.
On 1 June 2010, Fabio signed for A.S. Roma on a three year deal worth €1.8m per season, effective on 1 July 2010.[4] He scored 4 goals in this season. Also, he scored a winner against Lazio in Coppa Italia. Though only joining the previous year, during the summer of 2011 it was heavily speculated that he would be traded away in order to free up a spot for another non-EU player but no deals ever materialized.
He scored his fifth goal for Roma on October 1, 2011 against Atalanta. Roma won the game with result 3:1. Fabio scored his sixth goal for Roma on December 18, 2011 against Napoli in Naples, in a thrilling match. Roma won 1:3. After the game, he said that he wants to remain in AS Roma, and that he is very happy there. He played 6 games in first part of the season, and scored 2 goals.
On 17 November 2009, Fabio represented Brazil in a friendly match against Oman. He came in second half replacing Felipe Melo.
